- confirming that they meet the conditions for obtaining a certificate attesting to their knowledge of Polish at a given level of language proficiency.

In the case of completion of a form of education referred to in Article 11a(4)(2)(f) (a form of education conducted by a social organisation registered abroad or by another foreign entity in which teaching of the Polish language or other subjects in the Polish language is conducted abroad), the application for the certificate referred to in Article 11a(4) shall also be accompanied by a copy or digital representation of a certificate issued by the consul of the Republic of Poland competent with respect to the seat of the social organisation registered abroad or another foreign entity conducting that form of education, which confirms:

The basis for the issue of a certificate by a consul of the Republic of Poland [app.1 certificate of the consul] is the submission of 11 certificates of completion of consecutive school years.

In the event that the applicant does not have all school-leaving documents, it is necessary to enclose a certificate issued by the head of the school relevant to the period of schooling received [app.2 certificate of the school principal]. The headmaster's certificate will confirm that the applicant has been studying the Polish language or other subjects in Polish during the specified years. Without continuity of school certificates or in the absence of a certificate from the headmaster, the consul will not be able to issue the certificate necessary to apply for a certificate of proficiency in the Polish language without having to pass an examination.
